When to arrive
Please arrive about 15 minutes before your appointment so we can check your claim details.
Office hours are Monday through Friday, 8:30 AM to 5:00 PM Pacific time; closed weekends.
English and Spanish are spoken in our office.
What to bring
- A photo ID
- Your claim number, if you have one
- The name of your employer and your claims adjuster
- A list of any medications you are taking
- Your attorney’s details, if you are represented
